Annex A: Checklist in preparing a performance report

This checklist is intended as a high-level guide to assist integration authorities produce their annual performance report.

It does not represent an exhaustive list of required activities. Integration authorities are expected to build upon and tailor the checklist to their local needs and take into account the information presented throughout the rest of this guidance.

  • Ensure local analytical support is available to support the business as usual elements of performance reporting. Consider the additional support that may be required from PHS LIST analysts to produce the report
  • Consider the application of the Framework for Community Health and Social Care Integrated Services
  • Ensure that the report meets the content requirements
  • Ensure that an assessment of performance in relation to the national health and wellbeing outcomes is included in the report
  • Ensure that the report includes the integration authority’s performance against the national indicators (including a comparison between the reporting year and the five preceding reporting years)
  • Consider the use of wider data, supplementary to the integration indicators, to evaluate progress against the strategic plan
  • Ensure that the annual performance report contains information about the number of significant decisions that have been made outside of strategic planning and the reasons for such decisions
  • Ensure that detail is included on reasons for any review of the strategic plan conducted within the reporting period, whether this resulted in any changes and a description of changes
  • Ensure the report includes the required information for each locality
  • Ensure the report includes the required detail on any inspections carried out during the reporting year
  • Reflect on the output from the performance report, evaluating steps to rectify where ambitions are not being met and/or consolidate areas of success
  • Consider the publication approach (range of publication formats, potential launch events or presentations, promotion strategy, etc.)
