Examples to illustrate how to complete questions 4, 4a and 4b
Example 1
Application number 543, date of application 12/7/2018, has the following members:
Bill Smith Aged 35
Jill Smith Aged 30
Jane Smith Aged 15 (a child)
Application number 1043, date of application 12/7/2019, has the following members:
Bill Smith Aged 36
Jill Smith Aged 31
Jane Smith Aged 16 (no longer dependent - therefore recorded as an adult)
Assuming it is the same Bill, Jill and Jane Smith, and that none of them have applied since 12/7/2018, then questions 4, 4a and 4b (on application number 1043) would be completed as follows:
- question 4: 543 (application number of most recent associated application)
- question 4a: 0 (for no) – adults are not the same as Jane is now an adult
- question 4b: 0 (for no) – no longer children as part of application
Example 2
Application number 1543, date of application 12/6/2020, has the following member:
Jane Smith Aged 17 (a single adult)
Assuming it is the same Jane Smith that applied in July 2019, and that she has not applied since, then the answers to questions 4, 4a and 4b (on application number 1543) would be completed as follows:
- question 4: 1043 (application number of most recent associated application)
- question 4a: 0 (for no) – adults are not the same as Jane is single applicant
- question 4b: 1 (for yes) – no children on previous application nor this one
Example 3
Application number 1643, date of application 12/7/2020, has the following members:
Bill Smith Aged 37
Jill Smith Aged 32
Jane Smith Aged 17 (not dependent so recorded as an adult)
Again assuming it is the same Bill, Jill and Jane Smith who have already applied, and that none have applied since the application made by Jane on 12/6/2020, then questions 4, 4a and 4b (on application number 1643) would be completed as follows:
- question 4: 1543 (application number of most recent associated application)
- question 4a: 0 (for no) – adults are not the same
- question 4b: 1 (for yes) – no children on previous application nor this one
Example 4
Application number 1843, date of application 12/12/2020, has the following members:
Bill Smith Aged 37
Jill Smith Aged 32
Jane Smith Aged 17 (not dependent so recorded as an adult)
Tom Smith Aged 2 months
Again assuming these are the same Bill, Jill and Jane Smith who have already applied, and that none have applied since the application made on 12/7/2020, then questions 4, 4a and 4b (on application number 1843) would be completed as follows:
- question 4: 1643 (application number of most recent associated application)
- question 4a: 1 (for yes) – adults are the same
- question 4b: 0 (for no) – child(ren) now part of the application
Example 5
Application number 2843, date of application 12/12/2021, has the following members:
Bill Smith Aged 38
Jill Smith Aged 33
Jane Smith Aged 18 (not dependent so recorded as an adult)
Bob Brown Aged 20
Tom Smith Aged 1
John Brown Aged 3
Again assuming these are the same Smith’s who have already applied, and that none have applied since the application made on 12/12/2020, and that Bob and John have not applied before, then questions 4, 4a and 4b (on application number 2843) would be completed as follows:
- question 4: 1843 (application number of most recent associated application)
- question 4a: 0 (for no) – adults are not the same
- question 4b: 1 (for yes) – child(ren) still part of the application
