Title of Policy - Inclusion Health Action in General Practice (IHAGP)
Summary of aims and desired outcomes of Policy:
The IHAGP programme’s overarching aim is to tackle health inequalities (associated with socio-economic inequality) through specific interventions within general practice. It does this through actions/services which aim to:
- prevent health outcomes worsening for people with greatest need;
- proactively reach out to/engage those who maybe be ‘lost’ to the health system but need support to manage their health & social circumstances.
Desired outcomes:
- improved access for complex and high need patients;
- improved quality and continuity of care;
- staff have increased knowledge and confidence on how to recognise and address health inequity and therefore improve policy and practice;
- patients feel involved and empowered in their community and care.
