Maternity and neonatal (perinatal) adverse event review process: guidance
Operational guidance to support health boards boards undertaking perinatal adverse event reviews incorporating the additional reporting required of maternity services.
Introduction
This perinatal adverse event review guidance has been produced in consultation with Healthcare Improvement Scotland (HIS) and acts as an addendum to the HIS ‘Learning from adverse events through reporting and review’- A National Framework for Scotland. Stages 1 to 3 are consistent with the Framework and this document provides additional information that apply to stages 4 to 6 of the process.[1]
